I, the undersigned, wish to use the facilities and participate in activities provided by Iron Warrior Training Center, LLC. By signing this waiver, I acknowledge and agree to the following terms:

Assumption of Risk:
I acknowledge that using gym facilities, equipment, and participating in physical exercise involves inherent risks, including but not limited to physical injury, strain, discomfort, and the possibility of serious injury or death. I/We assume all risks and responsibility for any injuries or other medical incidents with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.

Waiver and Release:
I hereby release, waive, discharge, and agree not to sue Iron Warrior Training Center, its officers, employees, representatives, affiliates, or agents from any claims, demands, liabilities, rights, damages, expenses, and causes of action of any nature arising from my use of the gym facilities, whether caused by the negligence of the Provider or otherwise.

Medical Representation:
I represent that I am physically fit to use the gym facilities and participate in physical exercise. I have no medical condition that would prevent my safe participation. If I have any medical conditions or concerns, I have consulted with a healthcare provider and obtained clearance to participate.

Rules and Regulations:
I agree to abide by all rules, regulations, and policies of the Provider, including proper use of equipment, following safety guidelines, and respecting other members and staff. Specific gym rules are as follows:

• Proper attire, decorum and consideration of the rights and comforts of others must be observed at all times. 
• Beverages consumed during workout must be in a non-glass container with a lid.  Any spills MUST be cleaned up by the responsible party.
• Return all equipment to their respective places after use, including any removable plates from barbells/machines after use.  
• Equipment must be handled with care and proper/intended use at all times; any abuse will result in loss of fitness center privileges. This includes dropping "dumbbells" & "barbells", "crash pads" are provided if necessary. Equipment malfunctions must be reported immediately to theironwarriorla@gmail.com
• Please wipe off equipment after use.
• 20 minute limit per station, or allow members/guests to "work in" or share equipment.  Be courteous.
• Members/guests have the right to refuse being subjects in any video/photos used for social media or other use.  Please be respectful of others and their wishes.  If this request is not followed, management will handle accordingly.
• The distribution/sale of any prescription and/or illegal drugs/substances within the interior of Iron Warrior Training Center will not be tolerated and appropriate action will occur. This includes legal action if deemed necessary. 
• Let it be known, the only approved "changing area" or "dressing room" is the restroom/washroom.  Other areas of the facility are under video surveillance. 

24/7 Video Surveillance:
Iron Warrior Training Center is monitored 24/7 by surveillance cameras. Do not touch, move, or unplug any security cameras, failure to do so will result in immediate termination of membership/access rights.  This is to insure the safety of the members, guests, and staff.  The footage will be retained by Iron Warrior Training Center for subsequent review upon matters of membership status, liability, and/or property damage.

Management Membership Policy:
Iron Warrior Training Center, its officers, employees, representatives, affiliates, or agents have the right to revoke or refuse membership to any parties without cause. However, membership will not be revoked or refused due to race, religion, gender, national origin, or disability.

Consent to Medical Treatment:
I hereby consent to receive any necessary medical treatment resulting from my use of the gym facilities and agree to bear all costs associated with such treatment.

Acknowledgment:
I have read this Gym Waiver and Release Agreement, understand its contents, and agree to be bound by its terms. I understand that I am giving up substantial legal rights by signing this document.
